If you would like to request a name change, you will need to submit a Name Change Request Form, found on MyHU > Academics tab > E-forms > Name Change Form.
The following link will take you directly to the Name Change E-form on MyHU --> Name Change E-form. Login to MyHU is required to access the E-form via the provided link.
If this is a change to your legal name, please be prepared to provide documentation to our office so that we may verify your legal name. We will follow up with you via email regarding your request.
